Application of a Philadelphia Collar
A Philadelphia collar is a rigid cervical brace designed to immobilize the neck and provide support. Proper application is crucial for ensuring effectiveness and patient comfort.
Steps for Applying a Philadelphia Collar:
1. Preparation
Ensure the patient is in a neutral spine position (not flexed or extended).If the patient has a suspected cervical spine injury, application should be done with minimal movement and with the help of another person for stability.
2. Positioning the Back Piece
The posterior (back) piece is placed first.Slide the back piece gently behind the patient’s neck.Ensure the lower edge rests on the shoulders, and the upper part supports the lower skull (occipital area).
3. Positioning the Front Piece
The anterior (front) piece is placed next. Align the chin with the chin rest of the collar. Make sure the tracheal opening in the front is centered for airway access.
4. Securing the Collar
Fasten the Velcro straps or side fasteners evenly on both sides. Ensure a snug but comfortable fit—it should be tight enough to prevent movement but not so tight that it restricts breathing or circulation.
5. Final Adjustments
Check that the collar is symmetrical and properly aligned. Ensure the chin is properly positioned on the chin support. Verify there is no excessive pressure on the jaw or throat.
